Programs/Services » Assisted Living Apartment BuildingsHowe Sound is excited to announce the opening of our 20 unit apartment builiding in Burnaby for young adults with disabilities in Burnaby. The Surrey location is expected to open its door in June 2008.
LIBERTY PLACE- BURNABY
Each suite in the building will be completely self-contained with a bedroom, bathroom and kitchen, all fully accessible. Individuals will have access to around the clock staff, personal assistance, hospitality services and a 24-hour emergency response system. Fraser Health is in the process of assessing and referring individuals for suitability as residents. The residents must be capable of self-directing their own care and require a maximum of two prescribed services. Therefore, all residents must be capable of maintaining a certain level of independence in order to reside in these buildings. Assisted Living Eligibility - How is it determined?
Determining a person’s eligibility for Assisted Living is the responsibility of Fraser Health. If you are a current client of Home Health Services, discuss your care needs with your Case Manager and s/he will work with you to determine whether Assisted Living is the best option to meet your needs.
If you are not a current client you will need to be assessed by a case manager. Case Managers do not assess people specifically for Assisted Living, they assess people’s needs and determine with the person which services/supports would be most appropriate. You must be eligible for Home Health services to be eligible for Assisted Living. If you are having difficulty managing in your home, contact your local Home Health Office and ask for an assessment.
